CDU Rapid HIV test controls are only provided to approved Wyoming KnoWyo safety-net testing sites who have met the eligibility requirements. The KnoWyo Voucher Guidance, including eligibility criteria, can be found on the CDU Prevention Program website under the KnoWyo heading. 

• Copy of current CLIA Waiver
• Current CDU Surveillance data system training, access, and ROBs for all staff entering data into the data system
• CDU Rapid HCV tests are being correctly redeemed on the current KnoWyo Voucher platform
• CDU Rapid Test Training for each staff member performing CDU rapid tests
• CDU Rapid Test Agreement, KnoWyo Voucher Agreement, and KnoWyo Voucher Guidance Compliance Log
